Information for Our Patients and Prospective Patients

Each year, Turpin | Bradley Family Dentistry requires you to fill out new paperwork so we can update our records. This helps with many things, such as changes in medical history, medications, addresses, phone numbers, and insurance. 

Please click the button below to print out your patient registration forms. You may bring them to your appointment or email them to us at mtdds@live.com. 

If you have dental insurance, please bring it with you so we may make a copy. If you do not have a card, please be prepared with all insurance information so we can locate and file your dental insurance for you.

Turpin | Bradley Family Dentistry is NOT a Medicaid provider. We are an out-of-network provider.

We try to see all patients on an appointment basis and ask that you call in advance so we can reserve a time for you. We discourage walk-ins and ask that you call first so we can be better prepared to help you. 

Please call our office 24 hours in advance to reschedule your appointment. This courtesy makes it possible to give your appointment to another patient who may be experiencing a dental problem.
